Resumo: Introdução: As competências relevantes ao ensino médico atual são divididas em sete dimensões: cognitiva, técnica, contextual, integrativa, afetiva, relacional e hábitos da mente. No presente estudo, um instrumento de aprendizagem nomeado Registro Clínico Baseado em Problemas (RBP), composto por uma lista de problemas on-line dos pacientes com suas respectivas investigações e intervenções, foi desenvolvido e aplicado em um programa de residência médica em clínica médica. Objetivo: Este estudo teve como objetivo investigar a percepção dos médicos residentes e preceptores sobre a utilização do RBP no processo ensino-aprendizagem e no desenvolvimento de competências em suas várias dimensões. Método: Participaram do estudo 21 residentes e oito preceptores de uma enfermaria de clínica médica, distribuídos em três grupos focais, em que dialogaram sobre a utilização do RBP no serviço. Para a interpretação da fala dos participantes, utilizou-se a análise de conteúdo de Bardin. Resultados: Na percepção dos residentes, o RBP influenciou na organização do conhecimento e na motivação dentro da dimensão cognitiva. A dimensão integrativa foi a mais citada pelos participantes, visto que o RBP levou à reflexão e à estruturação do raciocínio por problemas, atuando positivamente na organização do conhecimento e na definição dos problemas mais relevantes. Na dimensão contextual, não houve consenso entre os residentes sobre o impacto na solicitação racional de exames, e os preceptores relacionaram a falta de impacto do RBP à falta de feedback. Na dimensão relacional, preceptores e residentes relataram que o RBP trabalhou a síntese e organização do pensamento. No discurso dos residentes, o RBP influenciou os hábitos da mente relacionados à capacidade do médico de autoavaliação e reflexão sobre sua prática. Conclusão: O RBP atuou como instrumento de aprendizagem, principalmente por estar associado a fatores psicopedagógicos relacionados à facilitação da aprendizagem. As dimensões de competências cognitiva, integrativa, contextual, relacional e hábitos da mente foram desenvolvidas pelo RBP. A visão dos residentes e preceptores sobre a relevância do RBP para a aprendizagem de competências foi divergente. As dimensões cognitiva, integrativa e hábitos da mente tiveram maior diferença entre os participantes. Para que o RBP gere um aprendizado mais eficaz, os preceptores precisam interagir mais com o instrumento e realizar regularmente o feedback com os residentes.

Abstract: Introduction: Competencies relevant to current medical education are classified into cognitive, technical, contextual, integrative, affective, relational, and habits of mind. In the present study, a learning instrument, called Problem-Based Clinical Record (RBP), composed of a list of patients’ online problems with their respective investigations and interventions was developed and applied in a Medical Residency Program in internal medicine. Objectives: To investigate the perception of resident physicians and tutors on the use of RBP in the teaching-learning process and the development of skills in its various dimensions. Method: 21 residents and 8 preceptors of a Medical Clinic ward participated in the study, distributed into 3 focus groups, where they discussed the use of RBP in the service. The participants’ oral accounts were interpreted using Bardin’s content analysis. Results: In the residents’ view, RBP influenced the organization of knowledge and motivation within the cognitive dimension. The integrative dimension was the most cited by the participants, since RBP led to reflection and the structuring of reasoning by problems, acting positively in the organization of knowledge and definition of relevant problems. In the contextual dimension, no consensus was found among residents regarding impact on the rational request for exams and the preceptors related to the lack of impact of RBP to the lack of feedback. In the relational dimension, preceptors and residents reported that RBP worked on the synthesis and organization of thought. The residents argued that RBP influenced habits of the mind, related to the physician’s ability to self-assess and reflect on his practice. Conclusion: RBP acted as a learning tool, mainly because it is associated with psycho-pedagogical factors related to the facilitation of learning. The dimensions of cognitive, integrative, contextual, relational, and habits of the mind were developed by the RBP. The view of residents and tutors on the relevance of RBP for learning skills was divergent. The cognitive, integrative, and habits of the mind dimensions had a greater difference between the participants. For RBP to generate more effective learning, preceptors need to interact more with the instrument and regularly provide feedback to residents.

Trata-se de um estudo de descritivo e prospectivo, realizado durante o atendimento farmacêutico nos ambulatórios de pós-transplante do Hospital Universitário Walter Cantídio da Universidade Federal do Ceará (HUWC/UFC), em Fortaleza-Ceará no período de abril a outubro de 2011. O presente trabalho objetiva apresentar as intervenções farmacêuticas realizadas em um serviço de Atenção Farmacêutica (ATENFAR) estruturado nos ambulatórios do transplante hepático e renal de um Hospital Universitário. As intervenções farmacêuticas (IF) foram classificadas de acordo com Sabater et al.(2005), a significância baseadas em Riba et al.(2000) e os Resultados Negativos associados a Medicamentos (RNM) fundamentados no Terceiro Consenso de Granada. As análises estatísticas foram realizadas no programa Epi Info v.3.5.1 e os testes de hipótese foram feitos no programa SigmaPlot v.10.0. O teste estatístico utilizado para análise da amostra foi o qui-quadrado (X²). Foram acompanhados 97 pacientes, identificados 54 problemas relacionados aos medicamentos e realizadas 139 intervenções farmacêuticas. As principais IF realizadas foram na educação do paciente sobre o tratamento (n=111; 80%) (p<0,05), já enquanto a significância todas as intervenções foram apropriadas, sendo que 83,4% (n=116) das IF realizadas no período do estudo mostram ser "significantes" (p<0,05). O farmacêutico, através do exercício da ATENFAR, é capaz de monitorar o tratamento farmacoterapêutico e intervir, quando necessário, integrando-se a equipe multiprofissional no cuidado ao paciente transplantado.

A descriptive and prospective study was conducted on the pharmaceutical care in the post-transplant outpatient clinic of Hospital Universitario Walter Cantidio of Universidade Federal do Ceará (HUWC/UFC), in Fortaleza- Ceará in the period of April to October of 2011. The aim of the present study was to describe the pharmaceutical interventions performed in a Pharmaceutical Care service structured in the liver and kidney transplant outpatient clinic of an academic hospital. The Pharmaceutical interventions (PI) were classified according to Sabater et al.(2005), with significance based on Riba et al.(2000) and the Negative Outcomes associated with Medication (NOM) established at the Third Consensus of Granada. Statistical analyses were performed using the Epi Info v.3.5.1 program and hypothesis tests were done with the SigmaPlot v.10.0 program. A chi-squared (X²) test was utilized for statistical analysis of the sample. A total of 97 patients were followed, where 54 problems related to medications were identified and 139 PI performed. The main PI were in education of the patient about treatment (n=111; 80%) (p<0.05), while the significance of all interventions were appropriate, where 83.4% (n=116) of PI performed in the study period were shown to be "significant" (p<0.05). Through pharmaceutical care, the pharmacist is capable of monitoring the pharmacotherapeutic treatment and intervening when necessary, while being part of the multiprofessional team caring for the transplant patient.

INTRODUÇÃO: A albumina pré-diálise pode ter sua utilidade questionada na avaliação do estado proteico devido ao efeito dilucional nesse período. OBJETIVOS: Avaliar se a albumina sérica dosada no período pós-diálise (pós-HD) seria um melhor marcador do estado nutricional e do risco de mortalidade, comparada à albumina pré-diálise. MÉTODOS: Investigou-se a correlação entre a albumina pré- e pós-HD e: o índice de massa corpórea (IMC), a adequação da circunferência muscular do braço (CMB) e da prega tricipital (PCT) ao percentil 50 (P50), proteína C-reativa ultrassensível (hs-PCR), o ângulo de fase (PA), o PNA (equivalente proteico do aparecimento de nitrogênio), o índice de adequação da diálise (Kt/V) e o estado de hidratação (correlação de Pearson). A concordância no diagnóstico do estado nutricional segundo a albumina pré- e pós-HD e o PA foi testada pelo coeficiente Kappa (K) (Bland-Altman). RESULTADOS: Foram incluídos 58 pacientes em hemodiálise (HD) (30 do sexo feminino, com idade média de 49 anos). O IMC, o PA e a hs-PCR apresentaram correlação significativa com a albumina pré- e pós-HD, enquanto a adequação da CMB ao P50 e o PNA o fizeram apenas com a albumina pós-HD. A concordância no diagnóstico de desnutrição, segundo o PA < 5 e albumina pré- e pós-HD < 3,2 g/dL foi regular (K = 0,432). Quando o ponto de corte da albumina para desnutrição foi de 3,7 g/dL (desnutrição leve ou risco de desnutrição), os diagnósticos foram concordantes somente no período pós-HD (K = 0,544). CONCLUSÃO: A albumina pós-diálise parece ser um melhor marcador do estado nutricional e de risco de mortalidade nos casos de desnutrição leve ou risco de desnutrição e nas situações de médio a baixo risco de mortalidade. O estado de hiper-hidratação pré-diálise pode representar um fator de confusão na interpretação clínica da albumina.

INTRODUCTION: Pre-dialysis albumin is likely to be falsely low due to a dilution effect, making its usefulness in assessing protein status questionable. OBJEVTIVES: The purpose of this study was to assess whether post-dialysis albumin would be a better marker of malnutrition and risk of mortality, when compared to pre-dialysis albumin. METHODS: We evaluated the correlation between pre- and post-dialysis albumin and the following parameters: body mass index (BMI), adequacy of muscle arm circumference (MAC) and tricipital skinfold (TS) to the 50th percentile (P50), C-reactive protein (CRP), phase angle (PA), protein equivalent of nitrogen appearance (PNA), the Kt/V index of dialysis adequacy, and the hydration status (Pearson's correlation coefficient). Agreement in the nutritional status according to pre- and post-dialysis (post-HD) albumin and PA was estimated according to the Kappa (K) coefficient (Bland-Altman). RESULTS: A total of 58 haemodialysis (HD) patients were included in this study (30 female; mean age: 49 years). BMI, PA and CRP had a significant correlation with pre- and post-HD albumin, while MAC and PNA correlated only with post-HD albumin. Agreement in the diagnosis of malnutrition according to PA < 5 and pre- and post-HD albumin < 3.2 g/dL was regular (K = 0.432). When using an albumin cut-off value of 3.7 g/dL for malnutrition (mild malnutrition or risk of malnutrition), the diagnosis was concordant only in the post-HD period (K = 0.544). CONCLUSIONS: Post-dialysis albumin levels may be a better marker of protein status and mortality risk in cases of mild malnutrition or risk of malnutrition and in patients with low/medium mortality risk. Pre-dialysis fluid overload may be a confounding factor when evaluating albumin levels.

A doença renal crônica (DRC) é uma síndrome clínica decorrente da perda lenta, progressiva e irreversível da taxa de filtração glomerular. A DRC pode apresentar várias manifestações bucais, como xerostomia, estomatite urêmica e periodontite, diagnosticada por meio da perda de inserção clínica (PIC). Foram avaliados 92 indivíduos em tratamento de hemodiálise na Clínica de Hemodiálise Prontorim, na cidade de Fortaleza, Ceará. O exame periodontal foi realizado por meio da mensuração da PIC e apenas 34 pacientes (37%) estavam aptos a realizar exame periodontal. A perda de inserção por indivíduo registrada no grupo de dentados ficou entre 1,31 e 5,27 mm, com média de 2,30 ± 0,96 mm. Dezoito pacientes (52,9%) apresentaram PIC menor que 2 mm, enquanto 16 (47,1%) mostraram-se portadores de periodontite. A perda dentária e a presença de considerável perda de inserção foram observadas neste estudo. Contudo, fatores como status social devem ser considerados em futuras investigações.

Chronic kidney failure (CKF) is a clinical syndrome due to the slow, progressive and irreversible loss of the glomerular filtration rate. CKF may be associated with several oral manifestations, such as xerostomia, uremic stomatitis and periodontitis, diagnosed as clinical attachment loss (CAL). 92 patients, from the Prontorim Hemodialysis Clinic of Fortaleza, Ceará, Brazil, were investigated. Only 34 patients (37%) were eligible for periodontal investigation, which consisted of CAL measurement (values grater than 2 mm diagnosing periodontitis). CAL measurements ranged from 1.31 to 5.27 mm (2.30 ± 0.96 mm). 18 (52.9%) patients had smaller than 2 mm CAL, while 16 (47.1%) had periodontitis. Tooth loss and considerable CAL were observed in this study. Nevertheless, factors such as social status should be considered in further investigations.
